Position: Home Health Aide Level 2 Classification: Hourly Reports to: Report to Branch Manager for administrative purpose and to the Clinical Manager for clinical competency. POSITION SUMMARY Performs personal care and related services in the home of clients. Adheres to and follows the Home Health Aide plan of service reflecting client/family/case manager request of care. ESSENTIAL FUNCTIONS Performs duties in the Service Plan as prepared. Responsible for assisting the client in completion of activities of daily living (ADLs) which include, but are not limited to: bathing, skin care, grooming, dressing, oral hygiene, and toileting needs. Observes and reports any changes in the patient condition. Assists the client with ambulation and transfers. May transfer clients using mechanical lifts. Plans and prepares nutritious meals for client as directed. Assists in the maintenance of a healthful and safe environment. Provides light housekeeping including laundering of clients garments and linens. Keeps care recipients active and engaged in daily activities of living. May remind the client and/or family to administer oral medications (ordinarily self-administered) as prescribed by physician. Documents all services provided and completes care logs for each shift. Communicates all changes in care recipient's physical or mental condition to the office including suspicions of exploitation, abuse or neglect. Submits care logs to the office as required. Maintains confidentiality regarding client information and all related documentation. Participates in mandatory company training and agency initiative and/or as required by office, federal and/or state regulations. Attends 12 hours of in-service per calendar year and mandatory agency meetings/trainings. KNOWLEDGE Has knowledge of: Elder care practice and procedures. How to handle care recipients with dementia or Alzheimer's disease. E-mail and cell phones. Knowledge of Agency's policies, procedures and operating guidelines relevant to employees in general and caregivers in particular. Stay current in the knowledge of their profession through continuing education. ABILITIES Speaks, reads and writes English. Able to follow verbal and written instructions and to document in writing services provided. Work independently in various care locations (private residence, nursing home, long-term care facilities) across the Alliance service area. Maintains caring attitude towards care recipients and their families even in difficult circumstances.
HHA Level 2 Maintains good communication with supervisors, and office staff through the regular use of a personal cell phone and email account. Demonstrates good interpersonal skills with tact and diplomacy. Refrain from gossiping. Time management -is able to complete plan of care during the time of the visit, manages travel time to arrive and depart on time for each assignment. Attention to details - complete care logs accurately and in a legible manner. Reports change in client's physical and mental condition. Problem solving - Recognizes rising issue. Takes appropriate step to resolve it on her own. Knows when to seek the support of the office staff or the clinical manager. WORK ENVIRONMENT Patient home setting and general office Exposure to infectious diseases Daily travel by automobile PHYSICAL DEMANDS Requires physical exertion such as assisting patients in transfer and ambulation. Regularly required to use hands and fingers, to handle, or feel, and talk or hear. Frequently required to stand, walk, reach with hands and arms, stoop, kneel, crouch, crawl or sit. Occasionally lift 20 pounds and/or move over 100 pounds with assistance of a lift device. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus. HOURS OF WORK This is an hourly paid position with variable hours. Employees in this position are expected to work at least 20 hours a week and every other weekend. Working on holidays is required. The employee's work schedule may change within his/her hours of availability to meet client needs. Client assignment may change from time to time. Minimum Requirement for the position 18 years of age or older. Must have a reliable source of transportation along with a valid driver's license and automobile insurance if driving while on the job or seeing multiple clients per day. Preferred Education and Experience At least one year of experience caring for seniors preferably as a Nursing Assistant. Licensed as a Certified Nursing Assistant High school diploma or GED.
