SUMMARY: A key component of this job is to support and promote the residents independence by establishing care goals; teaching the residents to understand their condition, medications, and self-care skills. DUTIES & RESPONSIBILITIES: + Assess the health needs of all recipients and make appropriate medical referrals as needed. + Participates in the development of residents Individual Support Plans (ISPs). + Maintains accurate, detailed reports and records symptoms or changes in residents? conditions. + Documents encounters with the residents on a progress note. + Interprets and evaluates diagnostic tests to identify and assess residents? condition and shares information with the team. + Consults and coordinates with residents? healthcare providers to assess, plan, implement or evaluate residents? care plans. + Monitors all aspects of residents? care, including diet and physical activity. + Facilitates groups for the residents on topics such as health education, disease prevention and assists the residents and the service staff to develop health improvement programs. + Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies. + Some evening and weekend work will be required. + Other duties as required or assigned by supervisor. EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS: Successful applicant must have strong written and verbal communication skills; must be able to provide good customer service, both in person and by telephone; must possess good organizational skills, have a high attention to detail, have the ability to handle multiple tasks, prioritize work assignments, and maintain confidentiality. Bi-lingual Spanish a plus. Must have at least 2 years of experience working with substance abuse, mental health and homeless and population. A bachelor's of science in nursing and current New York State RN License and Registration required.
